Surgeons at Duke University Hospital Implant first US Bioengineered Vein

A team of surgeons at Duke University Hospital helped create a bioengineered blood vessel and transplanted it in the arm of a patient with end stage kidney disease.  This is a milestone in the field of tissue engineering. The vein is engineered by cultivating donated human cells on a tubular scaffold to form a vessel.  The FDA recently approved a phase 1 trial 20 kidney dialysis patients in the US followed by safety review.

This process can significantly alter the lives of more than 350,000 U.S. hemodialysis patients and has potential to change procedures such as the coronary bypass procedure which is performed on more than 400,00 patients per year and other vascular grafts.

Case of the Month! by Dr. Kevin Brenner

78 year old male who presents from his dermatologist with a pigmented lesion of the left cheek which has been increasing in size over the past few months.  The patient’s dermatologist performed a shave biopsy which revealed malignant melanoma in-situ, lentigo maligna type.

What is the best option for further management at this point?

   A. Close monitoring with skin examinations every 6 months.

   B. Punch biopsy of the central core of the lesion.

   C. Excision of the entire pigmented lesion.

   D. Wide local excision with 0.5cm margins.

   E. Wide local excision with 2.0cm margins.

Surgical Procedures: Gallbladder removal through mouth

In 2010, UC San Diego performed a clinical trial operation where they removed a woman’s gallbladder through her mouth.  The surgery is called NOTES (natural orifice translumenal endoscopic surgery) which was done as part of a study to evaluate the safety and efficacy of the oral and transvaginal NOTES compared to laparoscopy, measuring primarily the differences in terms of pain levels, cosmetic outcomes and costs.

What were the biggest problems in early 19th century surgery?

The biggest problems in early surgery were:

  • Pain caused to the patient (the shock alone could kill some people)
  • Bleeding (patients could bleed to death very quickly)
  • Infection (blood poisoning and gangrene were common outcomes of surgery)

These problems were gradually solved in the 19th and 20th centuries.

  • Two American dental surgeons Horace Wells (1815-1848) and William Morton pioneered the use of nitrous oxide (laughing gas) and ether respectively as anesthetics (chemicals that stop someone feeling pain during an operation).  There are now many kinds of gases, vapors and drugs used to ensure a patient is comfortable during surgery.

    Joseph Lister in the UK developed ways to cut infection by sterilizing surgical instruments, hand washing and using a carbolic acid spray (see machine above) to kill germs in the air.  Nowadays, air in operating theaters is carefully filtered to remove bacteria and a germ free area around the site of the operation (a sterile field) is maintained at all times.  Modern antiseptics include betadine - the orange solution painted around operation sites prior to surger.  
  • Antibiotics such as Penicillin, developed in the 1930’s, mean few people nowadays die from blood poisoning and other infections.
  • Research into the nature of blood including the imporance of blood groups, led to successful blood transfusions in the early twentieth century.  These have reduced deaths from blood loss hugely.

Surgery Theater introduces Master Education Series for top surgeons

In an effort to push the envelope of progress, our productions team has created the Masters Educational Series. We have invited experts and opinion leaders from around the world to share their knowledge and techniques with aspiring surgeons, students, doctors and medical practitioners worldwide.

This series is divided into two parts: Masters Lectures and Masters Surgical Techniques. The first focuses on lectures, imparting a greater understanding of the subject as well as innovations in the field. The latter allows viewers to listen to the surgeon’s instructions as they watch his or her actions during surgery. It provides viewers the unique educational opportunity to watch a surgery as if they are in the operating room with the patient and surgeon!

We believe that global healthcare problems can only be overcome when education is the integral part of the solution. It is for this reason we invite experts to join our collaboration in an effort to create the ultimate portal for medical progress.

In order to participate in this series you must either be invited or nominated. After your invitation, we will arrange a convenient time to record a short interview and a lecture of your choice. For the Surgical Techniques series, we can either record the surgery or edit a prerecorded video of the operation with your voice over. Once the video is edited a live broadcast date on SurgeryTheater.com will be determined.
